You have tried a lot...have you tried a straight trojan scan or anything like that? It just seems very strange...also, you mention XP, did you update to SP2 during all of this? Could that be the problem? The only thing left that you haven't done is reformat the hard drive...and I'm sure you don't want to do that. If you installed XP SP2 when this happened..you may want to try to get rid of SP2 and see what happens...
