I believe the reason they are there, at least in Disney cartoons, is the result of animators simply being funny. There are so many animators on a film, that its practically impossible to track down who did the naughtiness, making it difficult to prevent. Disney doesn't put them there on purpose, they simply just slip through the cracks, and are usually cleaned up for the next release.

The head animator was a sick perv and slipped it all in last second, he got fired so you shouldn't find any of that stuff post-Pochahantas.
